Background:

The Ontario healthcare landscape continues to evolve, in a variety of ways including the establishment of Ontario Health Teams (OHTs) which are at various stages of formation and development. OHTs will improve the ability of healthcare professionals and their patients to navigate the continuum of care which today is quite siloed.

Connecting the siloes within the patient continuum (including funding) has long been identified as an enabler for healthcare providers/partners to work more efficiently and effectively together in support of patients and their families.

Products and services utilized within the patient’s journey will need to be coordinated from sourcing to receipt Healthcare supply chain professionals have both the opportunity and challenge to support their OHTs in securing what they need for patient care and, ensuring delivery to where it is needed.
